Beyond Happiness and Success: What Does Mental Health Really Mean?

World Mental Health Day Special | October 10

By Sonali Ojha

Clinical Psychologist and Researcher

Is mental health simply about finding inner peace, enjoying good health, maintaining physical fitness, achieving professional success, or attaining financial security? Or perhaps its deeper meaning lies in the extraordinary journeys of ordinary people confronting life’s most unexpected challenges. Ask ten people what mental health means, and you may receive ten different answers. For some, it represents professional accomplishment, for others it may be financial independence, emotional stability, healthy relationships, or a peaceful mind.

But do we truly understand mental health?

On this World Mental Health Day, I wish to share the journeys of two extraordinary women. They belong to different age groups, have different life experiences, and encounter entirely different crises. Yet their stories reveal something profoundly similar about the human mind.

When Life Changes in a Moment

The first woman was in her late twenties, brimming with youthful energy, enthusiasm, and dreams for the future. Life seemed full of possibilities, she embraced each day with optimism, confidence, and an infectious zest for living. Happily married to a caring husband, professionally accomplished, and surrounded by everything one might associate with a fulfilling life, she had every reason to look towards the future with optimism.

Then, on an otherwise ordinary day, the unimaginable happened. A sudden cerebral stroke left her hospitalized, critically ill, and confronting the frightening uncertainty of survival. Within hours, the life she had carefully built seemed to collapse. Her independence, physical abilities, and sense of control were threatened.

What followed was not merely a medical journey but an extraordinary psychological struggle. Fear, vulnerability, emotional distress, and uncertainty became part of her everyday existence. Recovery demanded much more than physical rehabilitation. It required psychological adaptation, emotional regulation, and the gradual restoration of self-efficacy, the belief in one’s ability to influence one’s circumstances. Her recovery was neither immediate nor linear. There were moments of helplessness and frustration. Yet, supported by her family, medical care, and her own determination, she gradually reclaimed her autonomy and identity.

Eventually, she transformed her experience into a book, documenting her journey from uncertainty to renewed purpose. Psychology recognises the possibility of post-traumatic growth, where individuals experience meaningful psychological changes following adversity. Such growth is neither automatic nor a requirement of recovery. For me, her journey represents an important dimension of mental health, that is the capacity to adapt, seek support, and reconstruct meaning when life becomes unexpectedly uncertain.

When a Mother’s World Falls Apart

The second woman was in her mid-forties, a confident individual, caring wife, loving mother, and someone enjoying a comfortable family life. Then came a diagnosis that shattered her psychological balance. Her child had developed a life-threatening malignancy. For a mother, that single word carried an unimaginable emotional burden. Suddenly, everyday concerns became insignificant beside one overwhelming question, what next? This was followed by sleepless nights, anticipatory anxiety, catastrophic thoughts, hypervigilance, and prolonged psychological distress. Each medical report carried emotional consequences. Every improvement brought hope, while every setback renewed uncertainty. The prolonged crisis placed enormous demands on her emotional resources. Yet she fought relentlessly, for four long years, she and her family navigated medical uncertainty, emotional exhaustion, and persistent fear. Their shared journey demonstrated the importance of familial resilience, adaptive coping, emotional co-regulation, and social connectedness. Eventually, her child recovered from the illness. Gradually, the mother began reclaiming her emotional equilibrium, adjusting to life beyond the prolonged medical crisis. She too decided to transform her experience into a book, a deeply personal account of suffering, survival, and renewed hope for others to look forward to, in distress. Her journey reminds us that psychological resilience is not always an individual achievement. Sometimes, it emerges through relationships, shared responsibilities, and the emotional strength of an entire family.

For me, this too is mental health.

Mental Health Is Not an Individual Island

These two women faced entirely different circumstances. One fought to reclaim her own life, the other struggled alongside her family to save her child. Yet both stories challenge our conventional understanding of psychological well-being. Mental health is not synonymous with constant happiness, uninterrupted success, or the absence of psychological distress. A person may experience anxiety, grief, uncertainty, or emotional exhaustion and still possess the capacity for resilience and recovery. Importantly, resilience does not mean remaining strong every moment. Crying, feeling overwhelmed, acknowledging vulnerability, and seeking professional psychological support are not signs of weakness.

From a biopsychosocial perspective, mental health emerges through the interaction of biological, psychological, and social factors. Our emotional well-being is influenced not only by our internal coping mechanisms but also by our families, relationships, communities, healthcare systems, and social environments. These women’s journeys demonstrate the significance of psychological flexibility, meaning-making, and supportive relationships in navigating adversity.

A Reflection on World Mental Health Day

As a psychologist, I believe we must broaden the conversation beyond individual happiness and personal success. Mental health is also about the environments we create for one another, the compassion we extend, the emotional safety we provide, and the support we offer when someone encounters circumstances beyond their control.

On this World Mental Health Day, perhaps we should ask ourselves a different question.

Not merely, “Am I mentally healthy?” But also, “Am I contributing to an environment where others can remain psychologically supported during their most difficult moments?”

Because mental health is not only about me. It is also about my family, my relationships, my community, and the world around me. Mental health is not the promise of a life without adversity. It is the possibility of finding support, dignity, resilience, and meaning even when life takes an unexpected turn.

And sometimes, the most powerful expression of mental health is not simply finding the strength to rise again, but discovering that we do not have to rise alone. Support each other,  spread love, kindness and compassion.
